5 Factor Battery Quiz

The 5 Factor Battery is one of the easiest and quickest ways to check whether you are experiencing symptoms of mental health conditions such as depression, anxiety, sleep disorders, OCD & PTSD.
Instruction : For each time, indicate how much you have been engaging in the behavior described over the past month. Take your time and answer truthfully for the most accurate results.
Feeling sad or empty or others observed you as sad or in tears.

Feeling disinterested or lack of pleasure in all or almost all activities

Significant weight loss or weight gain/change in appetite/eating habits.

Insomnia or oversleeping.

Being slowed down or restless than normal.

Fatigue or loss of energy.

Lack of concentration or focus.

Feelings of worthlessness or excessive or inappropriate guilt.

Reccurent suicide ideation or attempt.

Excessive anxiety and difficult to control worried thoughts.

Restlessness or feeling keyed up or on edge

Being easily fatigued.

Difficulty concentrating or mind going blank.

Irritability.

Muscle Tension.

Sleep Disturbance

Does the anxiety, worry, or physical symptoms cause clinically significant distress or impairment in social, occupational, or other important areas of functioning?

Disturbing and recurring feelings or thoughts causing distress or anxiety?

Ignore disturbing thoughts/images by involving into compulsive activities such as cleaning or re-checking things?

Repetitive behaviours such as washing hands, ordering, checking, praying, counting, chanting in silence, etc.

Repetitive/compulsive behaviour leads to temporary relieve from distress

Do you spend more than 1 hour everyday for these repetitive/compulsive behaviors?

Do you feel uncomfortable if objects like clothes/groceries or other things are not in perfect order?

Do you collect/save objects with no actual emotional value such as food boxes, wrapping papers etc?
